A clear record of completed bodywork

A massage invoice should confirm the service and its duration while keeping clinical notes private. This layout distinguishes a treatment session from an agreed add-on and a separately billed travel visit. It is a general business invoice, not a coded insurance claim, medical receipt or assurance that a payer will reimburse treatment.

How to use the template.

  1. Confirm the session duration from your appointment record. Do not substitute the full time a room was reserved for the treatment time billed.
  2. Describe add-ons as agreed services and check whether they were already included in the session price.
  3. Use the client's authorized billing details. Keep intake answers, assessment notes and treatment observations in the appropriate private record.
  4. Mark a session paid only after reconciling the payment you received. Edit the file, check the balance and retain the reviewed copy.

Useful answers.

Can I use this to claim insurance reimbursement?

It is not an insurance claim form or a specialized reimbursement document. Ask the payer what information and format it accepts, and obtain qualified advice on any applicable billing obligations.

Should a package purchase appear as individual sessions?

Describe what was actually sold. A package sale and a visit paid from existing package credit are different events; reconcile the credit record before billing the visit again.

Where do health notes belong?

Keep detailed health and treatment notes out of routine billing copy. Include only the service description needed for the invoice and share sensitive records through your practice's appropriate procedures.
