Record added project work before it starts
A change order connects a new request to an existing agreement. Write down what is being added, why the scope changed and whether the schedule needs revision before the team starts the extra work. This example covers an addition to a small fit-out only; its displayed total is the added-work amount, not the full revised contract value or an invoice.

| Description | Quantity / unit | Rate | Amount |
|---|---|---|---|
| Additional blocking behind the newly requested wall-mounted cabinet | 1 added scope | $160.00 | $160.00 |
| Extra cabinet support material to approved specification | 1 material lot | $75.00 | $75.00 |
| Additional installation and finishing time for approved change | 3 labor hours | $55.00 | $165.00 |
Total (USD): $400.00
Example: this fictional fit-out addition illustrates a change record, not construction cost guidance.
Displayed total: additional work in this example only; no original contract value, credit or tax is included.
[Reason for request and person requesting the change.]
[Original scope retained; added scope; affected drawing or specification revision.]
[Schedule effect, dependencies and revised completion agreement.]
[Original contract sum] [Earlier approved changes] [This change] [Revised contract sum: calculate and verify separately.]
[Authorized approval signatures, approval dates and attachments.]
For removed scope, credits or mixed additions and deductions, prepare a reviewed revision with the correct arithmetic instead of treating every item as an addition.
How to use the template.
- Identify the original agreement and the specific instruction or drawing that changed. Explain the difference in plain terms.
- Itemize the added work and state what remains included in the original scope. This sample does not calculate deductions.
- Discuss access, material lead times and schedule consequences; record an explicit agreement rather than assuming no time effect.
- Calculate the revised project value separately, check all prior changes and obtain the required approval through your own process before proceeding.
Useful answers.
Is the displayed total the revised contract sum?
No. It totals only the fictional added lines shown in this sample. Reconcile the original agreement, earlier approved changes, this adjustment and applicable tax separately to establish the revised value.
Can this form handle deleted work?
The sample is deliberately limited to additions. For credits or a mix of additions and deletions, revise the editable document and verify the net change and revised agreement totals carefully.
Does approving a change order also request payment?
Those are separate steps unless your reviewed agreement defines otherwise. Record scope approval here and use the agreed invoice process when payment becomes due.
Keep exploring.
